Allergy Test Pty. Ltd. as trustee for Perth Allergy Unit Trust trading as Perth Allergy (Perth Allergy) is committed to supporting and complying with the Privacy Act 1988 (Cth) (Privacy Act) and Australian Privacy Principles in the collection, use, access and storage of personal information we obtain as part of our services.

Collection of Personal Information

Personal Information is any information or an opinion about an individual or an individual who is reasonably identifiable from the information or opinion. Examples include an individual's name, address, contact number and email address. Perth Allergy collects personal information in carrying on business, including from our employees, such as tax file numbers, name, address and account information.

Collection of Sensitive Information

Perth Allergy is a health service provider for the purposes of the Privacy Act.

Special provisions apply for the collection of sensitive information. Sensitive Information includes the collection of “health information” for use by a health service. Examples of health information that Perth Allergy may collect include:

  • clinical notes of an individual's symptoms, diagnosis and the treatment given;
  • specialist reports and test results;
  • appointment and billing details;
  • prescriptions and other pharmaceutical purchases;
  • an individual's healthcare identifier when it is collected to provide to a health service; and
  • any other personal information (such as information about an individual's date of birth and gender) reasonably necessary to provide a health service.

Perth Allergy will generally collect personal information and sensitive information directly from you. Perth Allergy may also collect information from third parties as required.

You have no obligation to provide us with any information we request.

Use and Disclosure of Information

We respect your privacy.

The personal information you provide to Perth Allergy will be collected for the primary purpose of providing high quality health services.

We may use and disclose your personal information for a variety of purposes in connection with providing services to you, including to our information technology service providers and those who may host cloud-based storage of our data.

Direct marketing

We do not engage in direct marketing.

Storage and Security

We may collect, disclose and store information received from you in electronic or hard copy form. Perth Allergy will use all reasonable endeavours to ensure that:

  • it does so securely. In this regard, all records are password coded and all patient medical records are encrypted;
  • access to personal information is limited to Perth Allergy doctors and staff or third party medical professionals who reasonably require it; and
  • when the information is no longer required, we will destroy it or de-identify it to make it anonymous.

Website Cookies

We may use cookies to personalise and simplify your access to and use of our web site. A “cookie” is a small data file that is downloaded from our website and stored in a text file on your hard drive. The information is made up of a string of letters and numbers which uniquely identifies your computer and any username or password that you may have already registered on our website. Information is also generated and collected whenever a page is accessed on our website that records information such as time, date and specific page. We collect such information for statistical and maintenance purposes that enables us to continually evaluate our website performance. These cookies cannot be used to obtain any other information on you or on your hard drive unless you have already provided such information.

If at any time you do not wish to accept cookies or wish to delete existing cookies, you may manually configure your browser to refuse cookies or you may manually delete existing cookies from your hard drive. However, if you do so, you may not be able to use the full functionality of our website.

Accessing and Changing your Personal Information

You may request (in writing) access to personal information we hold about you. Access will be provided within a reasonable time.

We retain medical records of patients under the age of 21 years until they reach 30 years of age. For all other patients, records are retained for 10 years.

Complaints Resolution

If you have a concern about Perth Allergy's collection, use, storage or disclosure of your personal information please contact our Privacy Officer who will investigate and advise you of the outcome as soon as possible.

Amendments to this Privacy Policy

Perth Allergy may amend or update this privacy policy from time to time at its discretion.
